Key Codes in the Demo Data
The demo runs one site — Kalinda Ridge Mine (KAL01), open cut, Queensland — through one complete blast, with a second blast still on the drawing board.
| Code | What it is |
|---|---|
KAL01 |
The site. Areas NP (North Pit) → NP-480, NP-470, NP-RAMP, plus ROM |
BL-2026-00042 |
The blast the demo is about. North Pit 480 bench production blast, fired 14:12 on 15 July 2026. Stage RECONCILIATION, outcome PARTIAL_SUCCESS, not closed |
BL-2026-00043 |
The next one — stage DESIGN, design Rev A still DRAFT, approval PENDING |
BR-2026-00031 / -00032 |
The blast requests behind them |
Rev A → Rev B |
Design Rev A was REJECTED by the blast manager (row 3 too close to the final wall); Rev B pulled row 3 back to 15 m, cut the row-3 charge and lengthened the inter-row delay to 90 ms. Rev A is SUPERSEDED |
RA-2026-00042 |
Blast risk assessment — overall HIGH, residual MODERATE, six hazards from flyrock to unauthorised access |
NP480-001 … -012 |
The twelve holes, three rows of four |
NP480-007 |
The rejected hole. Collapsed at 4.2 m with 1.8 m of water → REDRILL_REQUIRED, never charged, skipped in the tie-in. Action BL-ACT-2026-00021 to redrill it is OVERDUE |
NP480-012 |
The misfired hole. Row 3 eastern extent; ~330 kg of emulsion left in the ground |
NP480-005 |
The deviation — took 412.5 kg against a planned 380 kg after a cavity. Approved, but the combined MIC across the delay window was never recalculated. This is the root cause of the vibration exceedance |
MAG-01 / MAG-02 |
Main explosives magazine and the separated detonator store |
EM-2606-118 |
The emulsion batch actually charged. EM-2604-061 sits WET in the quarantine cage |
EX-MOV-2026-000118 |
The sealed transport movement, magazine → bench, with a 3.5 kg item discrepancy |
CS-0042-01 |
The charging session — status VERIFIED, 11 holes charged |
MF-2026-0003 |
The misfire. Detected 14:44, isolated 14:50, treated over four steps, verified 12:15 next day, now CLOSED. Regulator notified |
GP-01 … GP-04 |
The four guard posts. GP-03 failed its initial radio check at 13:38 and confirmed clear at 13:52 on the backup channel — firing was held until it did |
BL-INC-2026-00007 |
Vibration exceedance — 9.4 mm/s at Kalinda Homestead against a 5.0 mm/s limit and a 4.1 mm/s prediction. Status ACTIVE, regulator notified |
BL-INC-2026-00008 |
The misfire incident — actual severity MODERATE, potential CRITICAL |
INV-2026-0014 |
The ICAM investigation. Root cause: deviation approval never recalculates MIC across the delay window |
BL-ACT-2026-00019 |
The action holding the whole blast open — resolve the 3.5 kg variance. AWAITING_VERIFICATION |
VM-01 / VM-02 / VM-03 |
Seismographs. VM-03's calibration lapsed 30 June 2026 — not deployed, but flagged as finding F3 |
AUD-EXP-2026 |
Annual surface explosives audit — 2 minor non-conformances, 1 observation |
The Blast, Step by Step
- 8 Jul —
BR-2026-00031raised for the 480 bench. - 10–14 Jul — Design Rev A rejected on geotechnical grounds → Rev B approved by geotech, environment (with a 190 kg MIC condition) and the blast manager. Risk assessment approved.
- 11 Jul — Twelve holes drilled.
NP480-007collapses at 4.2 m. - 12 Jul — Four holes surveyed; the 0.34 m deviation on
NP480-009accepted. - 15 Jul 06:30 — Condition inspections:
NP480-007fails, not for charging. - 15 Jul 06:45–07:32 — Explosives issued and transported to the bench under seal.
- 15 Jul 08:00–12:30 — Charging session.
NP480-005takes 412.5 kg (cavity); deviation approved. Stemming shortfall caught and corrected by quality check. - 15 Jul 13:10–13:52 — Clearance. GP-03 fails its radio check, then confirms.
- 15 Jul 13:55 — Pre-fire checklist: all fifteen checks pass, exceptions noted.
- 15 Jul 14:12 — FIRED. Shotfirer reports a possible unfired hole.
- 15 Jul 14:42 — Post-blast inspection: misfire indicators at
NP480-012.area_safe_for_reentry = false. No re-entry. - 15–16 Jul — Misfire isolated, risk-assessed, washed out (326.5 kg recovered), and independently verified.
- 16 Jul 12:30 — Second inspection clears the area → re-entry authorised.
- 17 Jul — Fragmentation P80 395 mm (target 350), muckpile good. Reconciliation shows a 3.5 kg variance → cannot be approved.
- 17–30 Jul — Investigation completes; six corrective actions raised.

Rules of Thumb
- A hole that fails its condition inspection is not charged — check the hole count in the reconciliation against the drilled count, not the planned count.
- Batch, not quantity, is the unit of custody. A suspect batch must be traceable to the exact holes it entered.
- A charge deviation is only half-checked if you verify the hole and not the maximum instantaneous charge across the delay window — that gap is exactly what caused the demo's vibration exceedance.
- Every guard post confirms clear before firing; one unconfirmed post blocks the shot, and lost communication suspends the clearance.
- A suspected misfire blocks re-entry. Treatment is verified by someone other than the person who performed it.
- A blast does not close with an unapproved reconciliation, an open misfire or an unexplained explosives variance.
- Authorisation expiry matters: the demo's drill supervisor authorisation expired
9 April 2026 and is still listed — audit finding
