  2. 10–14 Jul — Design Rev A rejected on geotechnical grounds → Rev B approved by geotech, environment (with a 190 kg MIC condition) and the blast manager. Risk assessment approved.

Rules of Thumb

  • A hole that fails its condition inspection is not charged — check the hole count in the reconciliation against the drilled count, not the planned count.
  • Batch, not quantity, is the unit of custody. A suspect batch must be traceable to the exact holes it entered.
  • A charge deviation is only half-checked if you verify the hole and not the maximum instantaneous charge across the delay window — that gap is exactly what caused the demo's vibration exceedance.
  • Every guard post confirms clear before firing; one unconfirmed post blocks the shot, and lost communication suspends the clearance.
  • A suspected misfire blocks re-entry. Treatment is verified by someone other than the person who performed it.
  • A blast does not close with an unapproved reconciliation, an open misfire or an unexplained explosives variance.
  • Authorisation expiry matters: the demo's drill supervisor authorisation expired 9 April 2026 and is still listed — audit finding AF-2026-03.
